Poor swing path
Quick or jerky tempo
Incorrect ball position
Incorrect stroke: such as the popular “mammoth backstroke jamming on the brakes on the through stroke”

Take the time to establish a consistent pre-putt routine for spot-on alignment.
Address the ball with a consistent set up. Recommended ball position is left of center.
For the shorter putts, a short backstroke and putter to the hole will keep the putter’s face more square longer for better rolls into the hole.
